2013 Great Smoky Mountain Classic - Entry Form

Please include our team in the 2013 Great Smoky Mountain Senior Classic tournament to be held on June 7-9 in Mayrville, TN. Deadline for entries is May 24, 2013. Rosters should be submitted with your entry form and fee (signatures can be added at the event). Enclosed is our check for the non-refundable entry fee of $400.00. All checks should be made payable to ISSA in US dollars.
